Clinic Schedule
Patient Applications
Monday & Wednesdays 2:00PM – 4:30PM
Applicants must bring proof of household income, have been a resident for a minimum of six months (Carroll, Grayson, or City of Galax) and be totally uninsured with no Medicaid, Medicare, or Veteran’s benefits.
